As our RN Case Manager, you'll orchestrate meaningful care that touches hearts and creates lasting comfort for families during their most vulnerable moments. This role goes beyond medical tasks—you'll be the steady, compassionate presence that makes all the difference.

• Lead with Heart: Guide a multidisciplinary team of LPNs, Aides, Social Workers, and Chaplains in creating personalized care experiences.
• Be the Advocate: Champion each patient's unique needs, preferences, and wishes while supporting their loved ones.
• Create Meaningful Connections: Build relationships that matter, providing counseling and emotional support when it's needed most.
• Make Every Day Count: Develop and implement care plans that prioritize quality of life and comfort.
• Tell Their Story: Document each patient's journey with care, capturing progress and precious moments.

As an RN Case Manager, you'll travel throughout Northeast Indiana, bringing healing and hope directly to patients in their homes, assisted living facilities, long-term care facilities, and hospitals—wherever they feel most comfortable.

Requirements

• Active RN License
• Valid Driver's License
• 1+ years hospice experience

Preferred Qualifications

• BLS Certification
• Hospice and Palliative Medicine background
