Responsibilities
- Leading with heart: Guiding a multidisciplinary team in creating personalized care experiences
- Being the advocate: Championing each patient's unique needs, preferences, and wishes while supporting their loved ones
- Creating meaningful connections: Building relationships that matter, providing counseling and emotional support when it's needed most
- Making every day count: Developing and implementing care plans that prioritize quality of life and comfort
- Telling their story: Documenting each patient's journey with care, capturing progress and precious moments
- Traveling throughout Miami/Darke counties to provide care in patients' homes, assisted living facilities, long-term care facilities, and hospitals
Requirements
- Active RN License
- Valid Driver's License
- 1+ years hospice experience
Qualifications
- BLS Certification
- Hospice and Palliative Medicine background
