As our RN Case Manager, you'll orchestrate meaningful care that touches hearts and creates lasting comfort for families during their most vulnerable moments. This role goes beyond medical tasks – you'll be the steady, compassionate presence that makes all the difference. You'll travel throughout the Youngstown area, bringing healing and hope directly to patients in their homes, assisted living facilities, long-term care facilities, and hospitals – wherever they feel most comfortable.

Responsibilities

• Lead with Heart: Guide a multidisciplinary team of LPNs, Aides, Social Workers, and Chaplains in creating personalized care experiences
• Be the Advocate: Champion each patient's unique needs, preferences, and wishes while supporting their loved ones
• Create Meaningful Connections: Build relationships that matter, providing counseling and emotional support when it's needed most
• Make Every Day Count: Develop and implement care plans that prioritize quality of life and comfort
• Tell Their Story: Document each patient's journey with care, capturing progress and precious moments

Required Qualifications

• Active RN License
• Valid Driver's License
• 1+ years hospice experience

Preferred Qualifications

• BLS Certification
• Hospice and Palliative Medicine background
