Job Summary
Athena Hospice of Rhode Island is seeking a part-time RN Case Manager for an on-the-road position with mileage reimbursement. The schedule is 8a-4:30p and services the entire state of Rhode Island. Compensation is $45 to $50 an hour, paid weekly.
Responsibilities
- Completes an initial, comprehensive, and ongoing assessment of the patient and family to determine hospice needs.
- Provides a complete physical assessment and history of current and previous illnesses.
- Supervises hospice aides.
- Provides professional nursing care by utilizing all elements of the nursing process.
- Affirms and evaluates the patient’s status by writing and initiating a plan of care, regularly re-evaluating patient and family/caregiver needs, and participating in revising the plan of care.
- Develops a care plan that establishes goals, based on nursing diagnosis, and incorporates palliative nursing actions. Includes the patient and the family in the planning process.
- Initiates appropriate preventive and rehabilitative nursing procedures.
- Administers medications and treatments as prescribed by the physician in the physician’s plan of care.
- Counsels the patient and family in meeting nursing and related needs.
Requirements
- Must possess an active and unencumbered RN license in Rhode Island.
- Must have a nursing degree from an accredited college, university, or school.
- Prior experience in hospice is required.
