Hospice RN Case Manager

About the role

Full‑time Registered Nurse position serving the Crawfordsville, IN area with Anew Hospice, a local family‑owned company operating exclusively in Indiana. The RN is responsible for delivering patient care through the nursing process of assessment, planning, implementation, and evaluation, and for planning, organizing, and directing hospice care services with an emphasis on community health education and experience.

Responsibilities

• Deliver patient care using the nursing process of assessment, planning, implementation, and evaluation.
• Plan, organize, and direct hospice care services for individuals and families in their homes and communities.
• Utilize community resources to meet the health needs of patients.
• Provide community health education and support.
• Collaborate with a close‑knit team that combines advanced technology with compassionate service.

Requirements

• Valid Indiana Registered Nurse (RN) license.
• Previous hospice or long‑term care experience preferred.

Schedule

Workday shift, Monday‑Friday schedule with nights free.
