Hospice RN Case Manager

About the role

The Registered Nurse (RN) at Anew Hospice is responsible for delivering patient care through the nursing process of assessment, planning, implementation, and evaluation. They plan, organize, and direct hospice care services and are experienced in nursing, with an emphasis on community health education/experience.

Responsibilities

• Deliver patient care through the nursing process of assessment, planning, implementation, and evaluation.
• Plan, organize, and direct hospice care services.
• Build from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.

Requirements

• Valid Indiana Registered Nurse License (RN).
• Previous hospice or long-term care experience preferred.

Qualifications

• Experience in nursing.
• Emphasis on community health education/experience.

Skills

• Strong communication skills.
• Ability to work independently and as part of a team.
• Knowledge of hospice care practices and regulations.
