Hospice Registered Nurse (RN) (PRN-Basis)

We are open to candidates who have an interest in Full-Time opportunities. The Registered Nurse Case Manager works Monday - Friday, daytime hours. Responsibilities

• Provide care and services for a group of patients, visiting patients where they reside (private homes, Skilled Nursing Facilities, assisted living facilities)
• Serve as a contributing member of the interdisciplinary team
• Provide expertise regarding psychosocial assessments, interventions, and knowledge of community resources

Requirements

• Current Registered Nurse (RN) licensure in good standing in the state(s) of practice
• Ability to work well within a team environment, collaborating daily with nurses, social workers, chaplains, volunteers, and CNA leadership
• Ability to work closely with patients one-on-one while receiving strong support from a patient care team
• Ability to work independently on a day-to-day basis, including phone, email, and written communication, meetings with healthcare professionals, and understanding of hospice rules and regulations
• Strong time management skills to manage visits, documentation, and meetings (schedule visits, attend required meetings, respond to urgent needs by prioritizing and triaging multiple concerns)
• Ability to learn an electronic medical records system
• Willingness to drive with reliable transportation, a valid driver’s license, and auto insurance to travel across the service area

Pay

$90,000–$95,000 per year plus bonus opportunities.
