Hospice Registered Nurse (RN) Case Manager

Responsibilities

  • Delivers patient/caregiver care, conducts nursing assessments, and manages care plans.
  • Facilitates patient and caregiver independence, applies patient-related protocols, and educates patients and families.
  • Communicates with interdisciplinary groups, supervisors, and external agencies to maintain continuity of care.
  • Maintains open lines of communication with supervisors and keeps them informed of complex patient/caregiver issues.
  • Applies infection control principles, follows universal precautions, and documents care accurately and timely.
  • Attends interdisciplinary group conferences and collaborates with team members to address patient needs.
  • Cleans patient rooms and maintains cleanliness and stock levels in the facility.
  • Uses protective equipment when necessary and ensures proper chemical usage during cleaning tasks.

Requirements

  • Current, active, and unrestricted compact (multistate) license as a Registered Nurse (RN).
  • Minimum 1-year related experience for inpatient units (IPU) and 2 years for other nursing positions.
  • Experience in direct care of patients with life-limiting illnesses, including hospice and palliative care.
  • Computer proficiency is required.

Qualifications

  • A Diploma or associate degree in nursing is required; a Bachelor’s Degree is preferred.
  • CPR certification is required; CHPN certification is preferred.

Compensation is competitive and commensurate with experience.
