Hospice Registered Nurse (RN Case) Manager

About the role

As a Hospice RN Case Manager at Trinity Health At Home, you are fully responsible for the client assessment, care planning, therapeutic intervention, and overall supervision of client care and outcomes. You are also designated as the case manager for hospice patients and collaborate with the physician and other Hospice team members to anticipate and manage patient’s symptoms, assist in identifying pt/family/caregiver grief/loss issues, and implement the Hospice plan of care.

Responsibilities

• Assess clients and develop individualized care plans
• Supervise and coordinate care provided by other healthcare professionals
• Collaborate with physicians and other team members to manage patient symptoms
• Identify and address grief and loss issues for patients and their families
• Implement and monitor the Hospice plan of care

Requirements

• Active RN license in Michigan
• 1+ years of clinical nursing experience (Hospice preferred)
• Strong communication, assessment, and organizational skills
• Certified in CPR
