Hospice Registered Nurse RN - Case Manager

Overview

Otterbein is a not-for-profit, faith-based ministry that is a national leader and innovator in retirement living communities and long-term care. Otterbein is seeking a Hospice RN Case Manager to assist in the growth of their Hospice ministry. Otterbein Hospice complements already strong independent senior services, outpatient capabilities, and clinical facilities at Otterbein.

Responsibilities

• This role is responsible for primary caseload.
• Provides skilled professional nursing care to patients, their families, and caregivers as prescribed by the physician and in compliance with the Hospice Medicare Conditions of Participation, Ohio Hospice licensure laws, and agency policies and procedures.

Qualifications

• Education: Bachelor’s degree in nursing preferred. Graduate of approved nursing program.
• Licensure/Certification: Valid Ohio Registered Nurse (RN) license.
• Experience: A minimum of 2 – 4 years’ direct patient care experience in hospice and Case Management experience preferred.
• Experience: Experience with end-of-life diagnosis preferred.
