Hospice Registered Nurse RN - Case Manager

About the role

Otterbein is seeking an enthusiastic and professional Hospice RN Case Manager to assist in the growth of our Hospice ministry. This role is responsible for a primary caseload, providing skilled professional nursing care to patients, their families, and caregivers as prescribed by the physician and in compliance with the Hospice Medicare Conditions of Participation, Ohio Hospice licensure laws, and agency policies and procedures.

Responsibilities

• Manage a primary caseload of hospice patients.
• Provide skilled professional nursing care to patients, families, and caregivers as prescribed by the physician.
• Ensure compliance with Hospice Medicare Conditions of Participation, Ohio Hospice licensure laws, and agency policies and procedures.

Requirements

• Bachelor’s degree in nursing preferred; graduate of an approved nursing program.
• Valid Ohio Registered Nurse (RN) license.
• A minimum of 2–4 years’ direct patient care experience in hospice; Case Management experience preferred.
• Experience with end-of-life diagnosis preferred.

Schedule

Full-Time, 8:00 AM–5:00 PM, Monday–Friday, plus on-call rotation as scheduled.
