Hospice Registered Nurse RN

About the Role

Primary functions are to administer skilled nursing care for clients of all ages in their place of residence, coordinate care with the interdisciplinary team, patient/family and referring agency, and assume responsibility for coordination of care.

Responsibilities

• Conduct initial and ongoing comprehensive assessments of the impact of the terminal disease on the patient’s physical, functional, psychosocial, and environmental needs and ADLs, including risk for grief, cultural/spiritual, and verbal/non-verbal communication.
• Implement the individualized Plan of Care (POC) and recommend revisions as necessary.
• Consult with and educate the patient/family regarding disease process, self-care techniques, end-of-life care, nutrition, and dietary needs.
• Provide training to other staff as needed.
• Initiate appropriate preventive and rehabilitative nursing procedures.
• Prepare clinical and progress notes demonstrating progress toward established goals.
• Coordinate all patient/family services and prioritize needs with members of the Interdisciplinary Team (IDT).
• Use a case management approach and refer to other services as needed.
• Inform physicians and other personnel of changes in the patient’s needs and outcomes of intervention, while evaluating patient/family response to care.
• Determine the scope and frequency of services needed based on acuity and patient/family needs.
• Assess the ability of the caregiver to meet the patient’s immediate needs upon admission and throughout care.
• Evaluate own needs for support and use identified systems to meet those needs.
• Communicate information using current processes and technology available to the organization.
• Apply specific criteria for admission and re-certification to hospice care to establish appropriate levels of care and patient eligibility.
• Perform on-call responsibilities and provide on-call service to patients/families as assigned.
• Other duties as assigned.

Qualifications

• Graduate of an accredited Associate or Bachelor’s degree in Nursing.
• Current state license as an RN and current state driver’s license.
• Two years’ experience as an RN in a clinical care setting; previous hospice or home health experience preferred.
• Current CPR certification.
• Proficient skill using a computer is required; Microsoft Office suite experience is preferred.
• Strong interpersonal skills and ability to provide effective support to the patient and family.
• Demonstrated skill in nursing practice, ability to cope with family emotional stress, and tolerance of individual lifestyles.
• Strong communication and organizational skills and a high standard of ethics and compliance.
