About the role
The Care Team Hospice is seeking a full-time Registered Nurse Case Manager to provide skilled nursing care and coordinated end-of-life support to patients in their homes and facility settings while collaborating closely with the interdisciplinary team.
Schedule, Hours & Territory
- Standard operating hours are Monday through Friday, 8:00 AM to 4:30 PM
- Crown Point, IN, and the surrounding communities within approximately a one-hour driving radius
- RN Case Managers may participate in an on-call rotation with additional pay
- Some branches offer dedicated on-call staff, resulting in backup on-call coverage only

Key Responsibilities
- Conduct comprehensive patient assessments and develop individualized plans of care
- Provide skilled nursing care in accordance with hospice standards and physician orders
- Coordinate care with physicians, caregivers, and interdisciplinary team members
- Educate patients and families on symptom management, disease progression, and end-of-life care
- Document assessments, interventions, and care plans accurately and in a timely manner in the EMR HCHB (Homecare Homebase)
- Participate in interdisciplinary team meetings and care planning
- Support after-hours patient needs as part of an on-call rotation when applicable
Qualifications
- Active Registered Nurse (RN) license
- Hospice or home health experience preferred
- Strong clinical assessment and care coordination skills
- Ability to work independently in the field
- Strong communication and documentation skills
- Experience using HCHB (Homecare Homebase) preferred
Field Role Requirements
- Active driver’s license
- Ability to pass a criminal background check
- Ability to pass a Motor Vehicle Record (MVR) check
- Current TB testing in accordance with agency policy
- Current CPR certification as required for the role
