Hospice Registered Nurse

About the Role

We are seeking a dedicated and compassionate part-time Hospice RN Case Manager to join our team. In this role, you will play a crucial role in enabling patients and their loved ones to live each day as fully and comfortably as possible. This is an on-the-road position with mileage reimbursement, covering the entire state of Rhode Island.

Responsibilities

• Conduct initial, comprehensive, and ongoing assessments of patients and families to determine hospice needs
• Provide professional nursing care by utilizing all elements of the nursing process
• Develop and implement individualized care plans that establish goals and incorporate palliative nursing actions
• Supervise hospice aides and provide guidance on patient care
• Collaborate with patients, families, and healthcare professionals to develop a comprehensive plan of care
• Provide emotional and spiritual support to patients and families

Requirements

• Active and unencumbered RN license in Rhode Island
• Nursing degree from an accredited college, university, or school
• Prior experience in hospice is preferred

The agency follows federal and state guidelines regarding staff vaccinations. Our policy requires all staff to be up to date with COVID and flu vaccinations, or acknowledge a declination of the vaccination(s).
