Responsibilities
- Provide compassionate, patient-centered care to individuals with life-limiting illnesses in home and facility settings
- Conduct comprehensive assessments and manage pain and symptoms
- Administer medications and treatments according to care plans
- Develop and coordinate individualized care plans in collaboration with an interdisciplinary team
- Educate and support patients and families to ensure understanding and comfort
- Document patient care accurately and maintain compliance with regulatory and quality standards
- Uphold dignity, comfort, and patient goals at the end of life
Qualifications and Skills
- Current Registered Nurse licensure
- Strong clinical assessment and pain management skills
- Excellent communication and patient support abilities
- Ability to work collaboratively within a multidisciplinary team
- Knowledge of regulatory and quality standards in hospice care
