As a Registered Nurse (RN) in Hospice, the work you do every day makes a difference in the lives of our patients. Our nurses give our patients the greatest gift – the ability to spend enhanced quality time with their loved ones in their preferred environment. As a RN, you will have the opportunity to provide one-on-one patient care and work at the top of your license. You will utilize your clinical skills in providing end-of-life care as directed by an attending physician. You'll be part of an interdisciplinary team that focuses on providing compassionate quality care to our patients.

How You'll Work

• Oversee the overall care of the patient from beginning to end of life
• Observe and monitor the patient to determine if the level of care being provided meets the patient’s needs
• Work autonomously and be responsible for all practices and duties within the scope of practice outlined by the state

Major Areas of Responsibility

• Plan of Care: Evaluate the treatment and medical condition of the patient according to the plan of care. Individualize the plan of care and make adjustments as needs change.
• Minimize Patient Risk: Update all doctor orders in the patient chart and administer medication as prescribed by the physician.
• Continuity of Care: Manage multi-disciplinary care as applicable while promoting continuity of care with appropriate admissions, transfers, and discharges.
• Family Educator/Advocate: Counsel patient and family on the disease/progression and incorporate patient and family in the development of the plan of care. Listen to patient and family members.
• Policies: Complete all clinical documentation following agency protocol and Medicare/Federal guidelines.
• Operations: Oversee and supervise the total care of the patient provided by nurse aides and LPNs. Attend in-service training and mandatory agency meetings.

Requirements

• Registered Nurse with current license in the state of employment
• Minimum one-year experience as an RN in an acute care setting is preferred
• Hospice experience is desired
• Valid driver's license and auto insurance in your name as a driver
• Capable of all physical demands

Qualifications

• Current CPR certification
• Knowledge of hospice care and end-of-life care

Schedule

PRN days/after-hours/weekends in the Akron/Canton area.
