This position is responsible for creating clear, accurate, and compliant admission narratives that establish hospice eligibility for patients with terminal illnesses. Responsibilities

• Write detailed and accurate admission narratives for patients with terminal illnesses, ensuring clarity and completeness.
• Collaborate with healthcare teams to gather relevant patient information and clinical details.
• Maintain confidentiality and adhere to all privacy regulations and institutional policies.
• Review and update narratives as needed to reflect ongoing patient status and care plans.
• Ensure all documentation meets organizational standards and regulatory requirements.
• Review patient clinical documentation, including history, diagnoses, and recent clinical notes.
• Write clear, concise, and compliant admission narratives that establish hospice eligibility based on terminal prognosis.
• Collaborate with hospice nurses, intake coordinators, and other team members to clarify clinical details as needed.
• Ensure all narratives meet regulatory standards and organizational policies.
• Complete assignments within agreed-upon turnaround times.

Requirements

• MD or DO with an active, unrestricted medical license.
• Knowledge of terminal illness care, palliative care, or hospice services is preferred.
• Strong understanding of Medicare hospice eligibility guidelines and terminal illness prognostication.
• Proven experience in clinical documentation, medical writing, or related fields.
• Strong attention to detail and organizational skills.
• Excellent written communication skills with a focus on clarity, empathy, and professionalism.
• Ability to work independently and manage time effectively.
• Ability to work independently and manage multiple documentation tasks efficiently.
