About the role

BridgingLife, a LifeBridge Health partner, is seeking a Part-Time (20 hours per week/0.5 FTE) Hospice Physician to join our Medical Director in providing care for hospice patients. This position offers the opportunity to work at any of our BridgingLife facilities, including Dove House in Westminster, Northwest Hospital in Randallstown, or Sinai Hospital in Baltimore.

Responsibilities

  • Attend and actively participate in weekly Interdisciplinary Group (IDG) meetings, providing clinical leadership, guidance, and medical expertise.
  • Certify and recertify patient eligibility for hospice care, considering all relevant clinical factors and documenting progression of disease.
  • Complete required narratives for certificates of terminal illness.
  • Ensure clinical findings support continued eligibility for hospice care prior to a patient's third and subsequent recertifications.
  • Participate in developing, implementing, and revising comprehensive plans of care (POC) for each patient, including pain and symptom management, medications, and measurable outcomes aligned with patient needs and rights.
  • Provide accurate and timely documentation of all patient care services.
  • Assume responsibility for medical care of hospice patients in conjunction with the attending physician and IDG, including writing prescriptions (including controlled substances) and signing death certificates.
  • Review medication regimens for effectiveness, side effects, drug interactions, and laboratory monitoring needs, assisting the IDG in defining and measuring meaningful outcomes.
  • Participate in an equitable share of on-call coverage, including overnight, weekend telephone coverage, and bedside rounding at inpatient facilities.
  • Confer with management and staff on complex care issues, resolving conflicts in patient care decisions with attending physicians as needed.
  • Optimize the comfort and dignity of each patient, providing care consistent with patient and family needs and goals.
  • Assist the Medical Director in managing inpatient and outpatient Palliative Care programs, with optional clinical or administrative work with palliative care teams.
  • Perform other reasonable duties as assigned by the Medical Director or designee.

Qualifications

Advanced knowledge and skills in hospice and palliative care medicine are desired for this role.
