The system includes seven acute care hospitals, including one of the largest academic and surgical hospitals in the northeast, and a large multi-specialty medical group.
Responsibilities
- Participate in the decision-making process in relation to admission to hospice at home or in the inpatient setting
- Participating as a member of the Interdisciplinary Team in the development of individualized care plans
- Certify (re-certify when necessary) patients eligible for hospice care under the Medicare Hospice Benefit
- Consult with attending physicians regarding pain and symptom management
- Cookordinate efforts with the attending physician to provide continuity of care
- Develop and monitor the medical component of the hospice program
- Participate in the development, coordination and implementation of hospice services and policies
- Conduct home visits (in person and virtual) as a consulting physician when appropriate
- Act as a hospice liaison and advocate with the national, state and local physicians, healthcare professionals, partners and community
- Perform related administrative functions as required and/or assigned

Schedule
Forty-eight (48) on call weeks per year: Monday 7am through Friday 7pm. Including mandatory in person attendance of 1 Inter Disciplinary Team Meeting (IDT) per week. Four (4) weekends per year: Friday 7pm through Monday 7am
