About the role
Hospice On Call Nurse positions are available at Interim Healthcare Hospice in Rock Hill, SC. These roles involve after-hour/weekend phone calls, PRN visits, admissions, and death visits.
Responsibilities
- Completes an initial/comprehensive assessment of the patient/family to determine hospice needs.
- Completes ongoing assessments of the patient’s physical and psychosocial status.
- Develops and revises a plan of care in collaboration with the Interdisciplinary Group (IDG), the patient/family, and the attending physician.
- Coordinates care among IDG members and other healthcare providers.
- Evaluates and coordinates the need for services provided to the patient/family.
- Administers medications and treatments as prescribed.
- Ensures availability of necessary equipment, supplies, drugs, and biologicals.
- Provides education and interventions to support the effective management of pain and other physical symptoms.
- Supervises LPN/LVN, Hospice Aide, and Homemaker assigned to the patient/family.
- Documents patient/family services accurately and timely.
- Attends Interdisciplinary Group meetings regularly.
- Participates in on-call duties as defined by the office on-call policy.
- Participates in the QAPI program.
- Performs other assignments as requested and assigned.
Requirements
- Graduate of an accredited school of nursing.
- Licensed to practice as a RN in the state in which employed.
- Twelve (12) months of accumulated experience as a registered nurse within the last five (5) years or evidence of current clinical knowledge.
- One (1) year of hospice care experience preferred.
- Able to provide proof of valid driver’s license and auto liability insurance.
- Able to provide proof of current CPR certification.

- Understands principles of pain/symptom management.
- Reads and interprets technical instructions related to the care of the patient.
- Knowledge and commitment to the hospice philosophy of care.
- Demonstrates excellent observation, verbal and written communication skills, problem-solving skills, basic math skills, and nursing skills per competency checklist.
- Able to work as a member of an interdisciplinary group.
- Able to effectively communicate with clinical and non-clinical employees, patients, and family/caregivers.
- Able to perform and prioritize multiple functions or tasks.
- Able to effectively deal with multiple changes.
- Meets applicable federal and state health requirements to provide patient care as appropriate.
- Meets the requirements of federal and/or state criminal and abuse background checks where applicable.

Benefits
Flat rate pay of $320/week plus additional compensation for visits, admissions, deaths.

Schedule
Available Monday-Thursday 5:00 pm - 8:00 am, Friday 5:00 pm through Monday 8:00 am.
