Responsibilities

  • Deliver patient care including personal care (ADL): bathing, body mechanics, nutrition, elimination, safety, special procedures, housekeeping services, and participation in interdisciplinary team conferences.
  • For NAs 1+4: Perform sterile dressing for wounds over 48 hours old; wound irrigation; ostomy care or ostomy irrigation; insertion of urinary catheter (replacement only); oxygen therapy (room setup and monitoring).
  • For NAs II: Break-up and remove fecal impaction; I.V. fluid-assisted activities; nutritional activities; suctioning (oropharyngeal, nasopharyngeal); tracheostomy care; insertion of urinary catheter (replacement only); sterile dressing for wounds over 48 hours old; wound irrigation; ostomy care or ostomy irrigation; oxygen therapy (room setup and monitoring).
  • Assume responsibility for establishing and maintaining effective working relationships with team members and identifying specific patient/caregiver needs, making referrals as needed.
  • Ensure accurate, complete, succinct, and timely clinical documentation in accordance with HPCCR guidelines and licensure/certification standards.
  • Demonstrate competency in knowledge and skills, maintain professional reputation, and stay updated on healthcare trends.
  • Clean patient rooms as required, ensuring cleanliness and proper stock of supplies, and keep family rooms clean throughout the day.

Qualifications

  • Minimum 1 year experience required.
  • Experience in home care, working with terminally ill, and/or geriatrics preferred.
