Comprehensive Community Hospice at Parker Jewish Institute is seeking a dedicated and compassionate full-time Nurse Practitioner to join our team. We want to speak with naturally caring, empathetic NPs who are focused on providing the best quality of end-of-life care for every patient.

About the role

The Hospice Nurse Practitioner will provide nursing care to terminally ill hospice patients, assist and support families in home care and bereavement, and collaborate with the hospice team to deliver compassionate care. Your role includes conducting patient encounters to determine initial and continued eligibility for hospice services.

The Parker Jewish Institute for Health Care and Rehabilitation, located on the Queens-Nassau County border in New Hyde Park, New York, is a non-profit facility offering inpatient programs such as sub-acute/short-term rehabilitation, long-term care, and nursing home care. Our community health services include certified home health care and a comprehensive hospice program serving terminally ill patients in their homes or nursing facilities, including Parker’s own nursing home.

Requirements

  • Current New York State license as a Nurse Practitioner (NP)
  • Degree from an accredited NP program
  • 1+ years of experience as an NP
  • Previous experience in hospice or home care (preferred)
  • Proficiency in computer technology and electronic medical records
  • Access to reliable transportation and willingness to conduct home visits
