Hospice Nurse Practitioner
Description
The Elizabeth Hospice Nurse Practitioner provides compassionate and experienced palliative care to patients with serious and life-limiting illnesses. Consults with the palliative care medical director to assess, develop, and implement a plan of care that aligns with the patient’s overall health status, goals, and values, while collaborating with the interdisciplinary team, patient, family, and community physicians. Responsible for carrying out the mission, vision, values, and commitment to quality of The Elizabeth Hospice. The Elizabeth Hospice is San Diego's oldest non-profit hospice of 45 years, serving all of San Diego County and South Riverside County with multiple hospice office locations.
Salary
$100,000 - $130,000; subject to experience and qualifications.
Schedule
Monday through Friday, 8:00 a.m. - 5:00 p.m.
On Call Requirement
Two on-call shifts required per month.

San Diego County, CA
Travel
Required for patient visits.
Requirements
- Must be licensed as a Registered Nurse by the State of California.
- Must be licensed as a Nurse Practitioner by the State of California.
- Must have Drug Enforcement Administration (DEA) Certificate.
- Must have valid National Provider Identification (NPI) Number or obtain within two (2) months of hire date.
- Must have successfully completed a Nurse Practitioner (NP) Program from a recognized university and obtained a Master of Science Degree in Nursing (MSN).
- National Certification required or obtain within two (2) months of hire date: American Association Nurse Practitioner (AANP) or American Nurses Credentialing Center (ANCC).
- CPR/BLS Card.
- Advanced Certification as Hospice and Palliative Care Nurse (ACHPN) required within one year of hire date.
- Requires a minimum of three (3) years nursing experience in a hospice or palliative care setting.
- Computer skills (word processing), use of adding machine, calculator, fax, photocopier.
- Bi-lingual (Spanish) preferred.
- Current California driver’s license and proof of insurance required.
