Hospice Nurse Practitioner

We’re seeking a compassionate and experienced Nurse Practitioner to provide essential support to patients and families across various care settings, including homes, long-term care facilities, and hospitals.

The Hospice and Palliative Care Nurse Practitioner (NP) functions independently as an advanced nursing professional, coordinating and assessing care needs, creating and modifying the plan of care, and making appropriate referrals for patients in collaboration with the hospice and palliative care interdisciplinary team. The NP works with the team to complete all regulatory requirements such as provider face-to-face, end-of-life visits, and assessments for re-certification. Under direct physician order or physician-approved protocols, the NP may independently order opioids, adjust dosages, and change other drugs as the patient’s condition requires. The role also includes on-call coverage rotation and serving as a primary resource for education related to hospice and palliative care.

Responsibilities

• Comply with organization-wide policies and procedures, including patient care requirements of all regulatory agencies.
• Conduct thorough medical, functional, and psychosocial assessments on assigned patients; perform and interpret diagnostic tests and procedures as defined by protocol.
• Assess whether a patient’s condition requires immediate physician attention and provide appropriate medical intervention when needed.
• Initiate and alter drug therapies, write orders according to established protocols or direct physician orders, and perform Face-to-Face, End-of-Life, and recertification assessment visits.
• Collaborate with the interdisciplinary team to provide continuity of care, recommend courses of treatment, and monitor ongoing care.
• Order IV therapy, including antibiotics and opioids, adjust dosages, and change drugs as the patient’s condition requires.
• Furnish medications per standardized procedures, including those available under the Providence St. Joseph Health formulary.
• Order feedings and placement/replacement of nasogastric or gastric tubes, adjusting rate and formula based on patient needs.
• Assist patients and families with decisions regarding goals, values, withdrawal of interventions, non-beneficial interventions, and resuscitation status.
• Educate patients and families regarding medical conditions, prognosis, and appropriate referrals to health facilities and community resources.
• Provide education to nursing staff, facility departments, and family members regarding the special needs of patients and the elderly population.
• Participate in interdisciplinary team meetings, document patient notes in the clinical medical record, and maintain accurate and timely records.
• Participate in provider on-call rotation and perform other duties as assigned.
• Establish and maintain courteous, cooperative relations with the public, patients, and other personnel.
• Participate in quality improvement activities and the development and implementation of education based on evidence-based practices.

Requirements

• Master’s Degree in Nursing (required).
• 2 years of experience in an acute or transitional care setting.
• Active and unencumbered RN license in the state of practice (required upon hire).
• National Provider BLS certification from the American Heart Association (required upon hire).

Skills

• Ability to work independently as an advanced nursing professional.

• Proficiency in conducting medical assessments, interpreting diagnostic tests, and adjusting treatments.
• Knowledge of regulatory requirements for hospice and palliative care.
• Ability to order and adjust medications, including opioids and IV therapies, within scope of practice.
• Excellent communication and collaboration skills with interdisciplinary teams, patients, and families.
• Experience in education and training for staff, patients, and families.
• Familiarity with hospital information systems and electronic medical records.

Pay

Compensation range: $65.87–$77.88 per hour (base pay). Additional compensation may be available for this role, such as shift differentials, standby/on-call pay, overtime, premiums, extra shift incentives, or bonus opportunities.
