Hospice Medical Social Worker (MSW)

As a Hospice Medical Social Worker, you’ll provide compassionate, patient-centered support in the comfort of patients’ homes, helping people and families navigate the emotional, social and practical challenges of illness and recovery. You’ll assess psychosocial needs, connect patients with community resources, support care planning and advocate for goals that promote dignity, stability and independence.

Responsibilities

Requirements
- Master's Degree from a school of social work accredited by the Council on Social Work Education
- State of Michigan Full Masters Social Work License Required
- Two (2) years of social work experience in health care setting
- Ability to provide service to acutely and chronically ill patients
- Ability to complete written, verbal, and observational tasks indigenous to the position
Qualifications

- Two (2) years of social work experience in health care setting
