Hospice LPN Continuous Care Nights

Schedule

  • Three 12-hour shifts per week
  • Includes every other weekend rotation
  • Four days off each week for improved work-life balance

Responsibilities

  • Provide direct patient care in accordance with the State Nurse Practice Act and hospice standards
  • Conduct comprehensive nursing assessments and implement individualized plans of care
  • Deliver skilled nursing services focused on comfort, dignity, and quality of life
  • Administer and monitor pain and symptom management interventions
  • Educate patients and families on disease progression, care plans, and symptom control
  • Document patient care accurately and timely within electronic medical records (EMR)
  • Collaborate with the interdisciplinary team to ensure coordinated, holistic care
  • Evaluate patient responses and adjust care plans as needed

Requirements

  • Graduate of an accredited Practical/Vocational Nursing (LPN/LVN) program
  • Current LPN/LVN license in the state of Ohio
  • Minimum 3 years of nursing experience (hospice, home health, med-surg, or community health preferred)
  • Current BLS certification
  • Strong clinical, communication, and organizational skills
  • Valid driver’s license, reliable transportation, and proof of insurance
  • Understanding of hospice philosophy and end-of-life care

Preferred Qualifications

  • Experience in hospice or palliative care
  • Familiarity with home-based care and EMR systems
