Key Responsibilities
- Provide direct patient care in accordance with the State Nurse Practice Act and hospice standards
- Conduct comprehensive nursing assessments and implement individualized plans of care
- Deliver skilled nursing services focused on comfort, dignity, and quality of life
- Administer and monitor pain and symptom management interventions
- Educate patients and families on disease progression, care plans, and symptom control
- Document patient care accurately and timely within electronic medical records (EMR)
- Collaborate with the interdisciplinary team to ensure coordinated, holistic care
- Evaluate patient responses and adjust care plans as needed
Required Qualifications
- Graduate of an accredited Practical/Vocational Nursing (LPN/LVN) program
- Current LPN/LVN license in the state of Ohio
- Minimum 3 years of nursing experience (hospice, home health, med-surg, or community health preferred)
- Current BLS certification
- Strong clinical, communication, and organizational skills
- Valid driver’s license, reliable transportation, and proof of insurance
- Understanding of hospice philosophy and end-of-life care
Preferred Qualifications
- Experience in hospice or palliative care
- Familiarity with home-based care and EMR systems

Schedule & Work-Life Balance
Three 12-hour shifts per week
Includes every other weekend rotation
