About the role
Provide compassionate end-of-life care in patients' homes or facility settings as a Hospice Licensed Practical Nurse (LPN).
Responsibilities
- Conduct patient assessments
- Develop care plans
- Deliver skilled nursing care per physician orders
- Coordinate with interdisciplinary teams
- Educate patients and families
- Document care accurately
- Participate in team meetings
Requirements
- Active LPN license
- Valid driver’s license
- Pass background and MVR checks
- Current TB testing
- CPR certification
Qualifications & Skills
- Clinical assessment and care coordination abilities
- Strong communication and documentation skills
- Experience with HCHB preferred

Schedule
Monday to Friday, with potential on-call duties supported by additional pay or backup coverage.
