About the role

As a Hospice Licensed Vocational Nurse, your voice to influence patient care is valued and empowered at every turn –whether through open, collaborative relationships with your direct manager or more formal opportunities through hospital councils and national nursing initiatives. You'll help shape decisions that elevate both patient outcomes and the future of nursing.

You'll provide compassionate, personalized support to patients receiving end-of-life care – often over the course of several weeks or months. You'll work with a dedicated multidisciplinary team who cares for patients like family. Together, you'll bring comfort, dignity, and guidance to patients and their loved ones throughout their hospice journey.

Responsibilities

  • Providing hands-on nursing care in the home or facility by managing symptoms, administering medications, and helping patients stay comfortable
  • Observing and reporting changes in patient condition to the RN and care team to ensure timely, coordinated care
  • Documenting patient status, care provided, and important updates clearly and promptly to ensure safety and continuity across the care team
  • Supporting patients and families with guidance, compassion, and practical education to prepare them for care routines, medications, and end-of-life care
  • Collaborating with the care team to provide smooth, patient-centered care

Qualifications

  • Graduate of an accredited LPN/LVN program
  • Minimum of one year experience as an LPN/LVN in a clinical care setting
  • Hospice Experience Preferred
  • Must be licensed as an LPN/LVN in good standing in the state(s) where they will be providing services
  • Meets all additional requirements for hire including state specific requirements
  • Current BCLS prior to delivering patient care
  • Reliable transportation and proof of valid automobile liability insurance
  • Must have valid driver’s license
