Hospice Licensed Practical Nurse (LPN)

Responsibilities

• Communicate frequently and accurately with registered nurses, supervisors, and other staff about the patient’s condition.
• Participate in case conferences and other immediate communication methods as needed.
• Document patient services accurately and promptly to reflect the plan of care.
• Provide patient and family/caregiver education and information relevant to the diagnosis and self-care plan.
• Administer medications and treatments as directed by the physician, adhering to nursing standards of practice.
• Coordinate hospice services, reporting identified needs to the interdisciplinary group as appropriate.
• Ensure availability of necessary equipment, supplies, drugs, biologicals, and other required items and services.
• Support the effective management of patient pain and other physical symptoms, assessing the patient/family’s response to interventions and treatments.

Requirements

• Graduate of an accredited Licensed Practical Nurse (LPN) program.
• Active license to practice as LPN/LVN in the state where hospice care is provided.
• Minimum one year of nursing experience.
• Hospice experience preferred.
