Responsibilities

  • Delivers patient/caregiver care by coordinating and managing care.
  • Conducts nursing assessments to manage patient/caregiver needs.
  • Develops, implements, evaluates, and updates the plan of care to ensure continuity and goal achievement.
  • Fosters patient and caregiver independence.
  • Applies VIA Health Partners protocols, standing orders, and guidelines when providing hospice care.
  • Provides education to patients and families as needed.
  • Communicates effectively with interdisciplinary groups, patients, caregivers, and other health professionals to maintain continuity of care.
  • Maintains open lines of communication with supervisors and keeps them informed of complex patient/caregiver issues.
  • Follows infection control principles and guidelines for universal precautions.
  • Attends and participates in interdisciplinary group conferences [IDG] and special care conferences.
  • Identifies and refers patient/caregiver needs to appropriate team members.
  • Ensures accurate, complete, succinct, and timely clinical documentation in accordance with VIA Health Partners guidelines and regulatory requirements.
  • Demonstrates competency in knowledge and skills, maintains professional reputation, and stays updated on healthcare trends.
  • Cleans patient rooms and ensures facility cleanliness and supply stock.
  • Keeps the family room clean throughout the day and uses protective equipment when necessary.

Requirements

  • A Diploma or Associate degree in Nursing is required; a Bachelor’s Degree is preferred.
  • A current, active, and unrestricted compact (multistate) license as a Registered Nurse (RN) is required.
  • CPR certification is required; CHPN certification is preferred.
  • Minimum 1 year related experience for inpatient units (IPU) and 2 years for other nursing positions.
  • Experience in direct care of patients with life-limiting illnesses is required.
  • Related experience may include hospice and palliative care, family medicine, internal medicine, geriatrics, oncology, home health care, and medical surgical fields.
  • Computer proficiency is required.

Qualifications
