Responsibilities
- Deliver patient care, including:
- Bathing (assisting, bed baths, tub baths, showers, sitz baths)
- Body mechanics (turning and positioning, transfers, use of lifts, ambulation, range of motion exercises)
- Nutrition (preparing for meal times, feeding patients, measuring intake and output, preparing snacks, assisting with meals)
- Elimination (bedpan/urinal, bowel/bladder retraining, specimen collection, perineal/catheter care, condom catheter application, douches, enemas, insertion of rectal tubes/flatus bags, emptying drainage devices, maintaining gastric suction)
- Safety (side rails/call rails, mitts and restraints, CPR/Heimlich maneuver, infection control, hand washing, isolation techniques, universal precautions)
- Special procedures (vital signs, pulse, respiratory rate, blood pressure, height and weight, application of heat/cold, decubitus ulcer care, surgical skin prep, dressing changes, ace bandages, TEDs and binders, postmortem care, cough/deep breathing)
- Housekeeping services necessary for patient/family/caregiver health and safety
- Participation in interdisciplinary team conferences (IDT) and special case conferences
- For NAs 1+4, additional responsibilities include:
- Wound care (sterile dressing for wounds over 48 hours old, wound irrigation)
- Ostomy care or ostomy irrigation
- Insertion of urinary catheter (limited to replacement once assessed by a licensed nurse)
- Oxygen therapy (room setup and monitoring of flow-rate)
- For NAs II, additional responsibilities include:
- Fecal impaction management
- I.V. fluid-assisted activities (assembling/flushing tubing, monitoring flow rate, site care/dressing change, discontinuing peripheral IV infusions)
- Nutritional activities (oral/nasogastric infusions, gastrostomy feedings, clamping tubes, removing oral/nasogastric feeding tubes)
- Suctioning (oropharyngeal, nasopharyngeal)
- Tracheostomy care
- Insertion of urinary catheter (limited to replacement once assessed by a licensed nurse)
- Oxygen therapy (room setup and monitoring of flow-rate)
- Additional responsibilities for both levels include:
- Establishing and maintaining effective working relationships with team members
- Identifying and referring specific patient/caregiver needs to appropriate team members
- Absolutely ensuring accurate, complete, succinct, and timely clinical documentation in accordance with HPCCR guidelines and licensure/certification standards
- Maintaining professional reputation and projecting VIA's professional image
- Maintaining professional development and staying updated on healthcare trends
- Maintaining current professional licensure, CPR certification, and CEU requirements
- Maintaining productive and professional relationships with external agencies and healthcare providers
- Managing time effectively while responding appropriately to crises
- Cleaning patient rooms and ensuring facility cleanliness
- Stocking facilities with appropriate supplies
- Ensuring family room cleanliness
- Using protective equipment when working with chemicals
- Minimum 1 year of experience required
- Experience in home care, working with terminally ill, and/or geriatrics preferred
