Hospice Home Care Case Manager (RN)

About the Role

Registered Nurse providing professional nursing care to patients and families in a home or alternate-site setting.

Responsibilities

  • Plan, deliver, supervise, and document nursing care based on assessments, physician orders, and policies.
  • Use professional judgment to individualize care plans, evaluate patient needs, and set realistic outcomes.
  • Assess educational needs, develop discharge plans, and provide patient/family education.
  • Participate in interdisciplinary team meetings and case conferences to ensure quality care.
  • Monitor symptoms, communicate findings, and intervene appropriately; document care accurately and timely.
  • Reconcile medications, educate to prevent adverse events, and observe reactions.
  • Delegate and supervise care given by aides, ensuring continuity and quality.
  • Maintain confidentiality, adhere to infection control standards, and promote a team-oriented environment.
  • Support professional development, participate in quality improvement, and assist in staff training.
  • Work primarily in patients' homes or skilled nursing facilities, requiring travel.
  • Participate in performance improvement and staff development activities, ensuring compliance with regulations and agency policies.
  • Maintain professional appearance and conduct.

Requirements & Qualifications

  • Associate Degree or Diploma in Nursing; BSN encouraged after two years of service.
  • Current North Carolina RN licensure or compact license.
  • Basic Life Support (BLS) certification.
  • Valid driver’s license and auto insurance.
