About the role
This role involves visiting patients in community and facility settings to perform physical and psychosocial assessments, documenting visits electronically. The Hospice Field Case Manager develops and implements nursing care plans, making recommendations based on patient needs. Responsibilities
- Conducting patient assessments in diverse settings
- Developing personalized care plans
- Making care recommendations to ensure quality patient outcomes
- Participating in backup On Call duties 2-3 times per month
- Providing compassionate, patient-centered care
Qualifications
- Associates/Diploma in Nursing; BSN preferred
- 2+ years’ experience in med/surg, community health, or oncology nursing preferred
- NYS RN license and driver’s license required
