Hospice Field RN Case Manager

About the role

As a Hospice Field Case Manager, you will visit patients in community and facility settings to perform physical and psychosocial assessments, document visits electronically, develop nursing care plans, and make care recommendations based on patient needs.

Responsibilities

• Conduct patient assessments
• Create and implement care plans
• Provide care recommendations
• Participate in a backup On Call rotation 2–3 times per month
• Work one weekend per month
• Car and escort services available for On Call shifts

Requirements

• Associates or Diploma in Nursing (BSN preferred)
• Current NYS RN license
• Valid NYS Driver’s license
• 2+ years of experience in med/surg, community health, or oncology nursing (preferred)

Skills

• Strong assessment and care-planning abilities
• Proficient electronic documentation
• Ability to work independently in diverse settings
