Hospice Field RN Case Manager

About the Role

This field-based role involves visiting patients to perform physical and psychosocial assessments, documenting visits electronically, and developing nursing care plans. Visits occur in both community and facility settings. The Hospice Field Case Manager makes care recommendations based on patient needs and may serve as a backup on-call provider 2-3 times per month, with transportation support available.

Requirements

• Associate’s/Diploma in Nursing; BSN preferred
• 2+ years in med/surg, community health, or oncology nursing preferred
• Valid NYS RN license and driver’s license
