Hospice Field RN Case Manager

About the Role

As a Hospice Field Case Manager, you will perform physical and psychosocial assessments of patients in community and facility settings, electronically document visits, develop and implement nursing care plans, and make care recommendations based on patient needs. This role emphasizes delivering high-quality, patient-centered care with a focus on compassion and respect.

Responsibilities

• Visit patients, document encounters, and develop care plans.
• Coordinate care delivery based on patient needs.
• Participate in backup On Call duties 2-3 times per month (car and escort service provided).

Requirements

• Associate’s/Diploma in Nursing; BSN preferred.
• 2+ years’ experience in med/surg, community health, or oncology nursing preferred.
• Active NYS RN and Driver’s licenses.
