About the role
As a Hospice Field Case Manager, you will visit patients in community and facility settings to conduct physical and psychosocial assessments, document visits electronically, develop nursing care plans, and make care recommendations based on patient needs.
You will work one weekend per month and serve as a backup on-call 2-3 times monthly. Car and escort services are available for on-call shifts.
Responsibilities

Requirements
- Active RN license in the state
- Associates or Diploma in Nursing (BSN preferred)
- 2+ years of experience in med/surg, community health, or oncology nursing
- Valid NYS driver’s license
