Key Responsibilities
- Provide skilled bedside nursing care following the patient’s plan of care and under RN supervision.
- Monitor patients actively, report changes in condition, and manage acute medical symptoms to achieve palliation during crisis periods.
- Administer medications, monitor vital signs, and evaluate patient and family needs.
- Assist with plan of care preparation and execution, including infection control and symptom management.
- Document patient care activities and communicate findings with interdisciplinary teams and physicians.
- Participate in bereavement support and may assist with death pronouncements as allowed by state law.
- Provide guidance to hospice aides under RN supervision as per Nurse Practice Act regulations.
- Participate in call schedules and provide on-call nursing support as directed.
Qualifications
- Licensed Practical/Vocational Nurse (LPN/LVN) licensed in the state of employment.
- CPR certification required.
- Valid driver’s license and current automobile insurance.
- Graduate of an accredited Practical Nursing program.
- Minimum of 2 years’ experience as an LPN/LVN in hospice, home health, or long-term care; 3–5 years preferred.
Skills And Knowledge
- Strong knowledge of hospice patient needs, terminal illness care, and interdisciplinary team roles.
- Excellent patient assessment and communication skills.
- Understanding of regulatory guidelines such as Medicare, Medicaid, and ACHC.
- Ability to provide compassionate support to patients and families during end-of-life care.
- Effective documentation and program management skills.

- Flexible and adaptable, able to manage multiple tasks efficiently.
- Professional, mature, and cooperative with strong interpersonal skills.
Tools and Equipment
- Proficient with nursing assessment tools, computers, fax, copier, and communication devices.
- Reliable transportation required for travel between patient locations.
