About the role

Provide each assigned patient with personal care services in accordance with the patient’s nursing assessments and care plan, and as directed by nurse supervisors.

Responsibilities

• Steward the philosophy and mission of Patriot Healthcare, focusing on human dignity and respectful response to the needs of persons with life-threatening, terminal, and/or chronic illnesses.
• Participate with the Interdisciplinary Group (IDG) in the development and implementation of the patient/family’s plan of care.
• Perform all assigned tasks in accordance with Hospice policies and procedures, and as instructed by supervisor.
• Follow work assignments and/or work schedules in completing and performing assigned tasks.
• Ensure patient comfort at all times, including changing bed linens, assisting with daily care (mouth care, bathing, dressing, hair care, nail care, shaving, toileting, feeding), and providing back rubs.
• Assist with exercises, ambulation, and transferring patients (e.g., into and out of beds, chairs, bathtubs, wheelchairs, lifts).
• Apply elastic stockings, heel/elbow protectors, and assist with special treatments (e.g., indwelling catheter care, perineal care, nonsterile vaginal irrigation/douches, bandage application).
• Monitor and document intake/output records, bowel movements, and skin condition; report pressure areas, skin breakdown, or injuries of unknown source.
• Provide care for the dying patient and post-mortem care as instructed.
• Document tasks performed and visit-related comments.
• Communicate with family or facility staff to ensure nursing services meet patient needs.
• Maintain a warm, calm, and positive environment for the patient.
• Report complaints, grievances, or areas of concern to supervisors.
