Responsibilities

• Provide personal care and emotional support to hospice patients in Skilled Nursing Facilities (SNFs) and private residences
• Assist with activities of daily living (bathing, grooming, toileting, transfers, feeding)
• Observe and report changes in patient condition to the clinical team
• Support families with education and comfort measures
• Maintain accurate documentation per hospice and regulatory guidelines
• Work collaboratively with nurses, social workers, chaplains, and the interdisciplinary team
• Maintain patient dignity, privacy, and comfort at all times
• Travel throughout the service area daily

Qualifications

• Current New Mexico CNA license
• Hospice or home health experience preferred
• Valid driver’s license, reliable transportation, and auto insurance
• Able to travel daily within service area
• Strong communication skills and compassionate bedside manner
• Able to work independently and as part of a team
• Must pass background check and drug screening
