Hospice Clinical Team Manager (RN Required)

About the role

The Team Manager leads the Patient Care Team, overseeing all patient care services including volunteer and bereavement support. They also manage the interdisciplinary team, ensuring that services are aligned with the evolving needs of patients and families.

Responsibilities

• Supervise, evaluate, and coordinate the interdisciplinary team
• Evaluate and approve patient admissions and discharges
• Collaborate with physicians on palliative care measures
• Ensure accurate and complete patient records
• Manage team schedules and services
• Facilitate interdisciplinary meetings
• Participate in on-call rotations
• Interview, select, train, supervise, and dismiss staff
• Oversee staff and volunteer schedules
• Review and approve payroll
• Monitor clinical records and team activities
• Develop and implement performance improvement activities
• Assure completion of patient/family satisfaction surveys
• Perform substantive chart reviews
• Participate in outcomes management and annual program review
• Conduct utilization reviews
• Address customer service issues and grievances
• Provide inservices to customers
• Verify and approve patient care services
• Control and be accountable for productivity and costs

Qualifications

• Reliable transportation with appropriate license and insurance
• Two years successful supervisory experience or equivalent in a health care organization
• Bachelor's degree preferred
• Current and valid R.N. License

Skills

• Knowledge of primary medical and nursing care principles
• Understanding of interdisciplinary health care team dynamics
• Working knowledge of VITAS information system
• Knowledge of the process of dying and bereavement
