As a Chaplain the work you do every day makes a difference in the lives of our patients. Our team gives our patients the greatest gift – the ability to spend enhanced quality time with their loved ones in their preferred environment. You will be part of an interdisciplinary team that focuses on providing compassionate quality care to our patients.
Responsibilities
- Provide direct spiritual care to patients, families, and caregivers.
- Assure spiritual assessment of patients and families/caregivers in the hospice program and appropriate services are rendered in a timely manner.
- Facilitate the development in the interdisciplinary group plan of care to meet identified spiritual needs.
- Maintain records of spiritual care services utilization and related activities for quality assurance, program development, and policies and procedures review and revision.
- Document direct services and ongoing communication with community chaplains and spiritual counselors.
- Provide funeral or memorial services for patients as requested.
- Plan periodic memorial services to meet the needs of personnel, volunteers, and community clergy/spiritual counselors working with the hospice team.
- Provide consultation, education, and support to the interdisciplinary group on spiritual care.
- Assist in supervision of spiritual care volunteers when assigned to patients/families/caregivers.
Requirements
- Graduate of accredited seminary or school of theology (BD, MDiv or equivalent theological degree) preferred.
- Experience working with death and dying, individuals/family/caregiver.
- Knowledge of and commitment to hospice philosophy of care.
- Hospice experience preferred.
- Valid driver's license and auto insurance in your name as a driver.
- Capable of all physical demands.

Schedule
Mon - Fri 8 am to 4:30 pm as needed (PRN).
