About the Role

Full-time Hospice Chaplain position covering 1-2 counties surrounding the office. This role involves providing spiritual care and support to terminally ill patients and their families, coordinating with healthcare teams, and participating in bereavement programs.

Responsibilities

• Perform accurate and comprehensive spiritual assessments within 5 days of the start of care.
• Ensure timely communication of changes in patients’ clinical status to healthcare team members and office staff, both verbally and in writing.
• Coordinate care planning and delivery with physicians, patients/families, and healthcare teams to optimize patient outcomes.
• Act as a liaison between Hospice and the spiritual community.
• Respond to the mental, spiritual, social, and emotional needs of terminally ill patients and their families.
• Provide nondenominational spiritual counseling per patient and/or family request.
• Involve patient/family in the formation of the plan of care and interact professionally with them.
• Communicate clearly with physicians, staff, Clinical Nurse Manager, and administrative teams.
• Assist families in understanding the grief process and provide referrals to community grief support groups.
• Participate in the bereavement program, including facilitating grief support groups, leading funeral services, providing bereavement counseling, and notifying staff of funeral services.
• Allow patients and families to voice concerns, fears, and feelings, and address them appropriately.
• Support and respect cultural practices that do not harm others.
• Support a culture of safety and quality, and comply with all relevant policies, laws, and regulations.
• Perform specific religious duties related to the faith in which you are ordained (e.g., administration of last rites, baptism).
• Understand Hospice philosophy and issues related to death and dying.
• Adhere to documentation timeliness policies and professional licensure standards.
• Assist physicians and healthcare teams in understanding spiritual, emotional, and social factors related to patient health to foster spiritual well-being and human dignity.
• Treat patients and families with respect and dignity.
• Participate in the on-call rotation.

Requirements

• Master’s Degree in Ministry, Counseling, or Social Work required.
• Current driver’s license, reliable transportation, and insurance.
• One year of Hospice experience preferred but not required.
