Hospice Case Manager-RN, Full time

BAYADA Hospice is looking for a compassionate and dedicated Full Time Registered Nurse (RN) to join our team in the Brattleboro, VT office. This role services hospice clients on a per visit basis in Wyndham and Windsor counties in VT and Cheshire, Sullivan counties in NH, with a coverage area including Springfield and Claremont.

About the role

As a hospice nurse (RN), you will be an integral member of a multi-disciplinary health care team that provides palliative care to clients, enabling them to receive medical care at home. Prior home care and/or hospice experience is strongly preferred but not required.

Responsibilities

• Making home visits to clients in designated geographic territories.
• Performing assigned duties, including administration of medication, wound care, treatments, procedures, and comfort care.
• Monitoring clients' conditions; reporting changes to Clinical or Client Services Manager.
• Following up with, executing, and properly documenting doctors' orders.
• Performing client assessments as necessary.
• Occasional on-call shifts after hours and/or weekends.

Requirements

• A current license as a Registered Nurse (RN) in VT & NH.
• A minimum of one year of recent, verifiable clinical (medical/surgical) experience.
• Solid computer skills; prior experience with electronic medical records (EMR) preferred.
• Ability to travel to cases as assigned.

Pay

Base pay: $45–$55 per hour, depending on qualifications and subject to negotiation. $10,000 sign-on bonus.
