About the Role

The Registered Nurse Case Manager (RN) provides patient care in various settings, including private homes, Skilled Nursing Facilities, and assisted living facilities, as part of an interdisciplinary team.

Responsibilities

• Deliver clinical care and conduct psychosocial assessments
• Coordinate interventions while managing a caseload of patients
• Communicate effectively via phone, email, and meetings
• Ensure documentation and adherence to hospice regulations
• Visit patients at their residences
• Collaborate with healthcare professionals
• Prioritize urgent needs efficiently

Requirements

• Current RN licensure in Georgia
• Hospice experience
• Strong teamwork and independent work ability
• Excellent time management skills
• Proficiency with electronic medical records
• Reliable transportation, valid driver’s license, and auto insurance
