Hospice Case Manager Registered Nurse, RN

About the role

We are seeking a Registered Nurse Case Manager to join our exceptional clinical care team at Suncrest/Brighton Hospice in the Atlanta area. This position offers competitive pay starting at $85k-95k, a generous benefits package, and the opportunity for career growth within a rapidly expanding national company.

Responsibilities

• Provide care and services for a group of patients and visit patients in their homes, Skilled Nursing Facilities, and assisted living facilities.
• Collaborate with the interdisciplinary team to provide psychosocial assessments, interventions, and knowledge of community resources.
• Work independently and on a 1-1 level with patients while receiving support from a patient care team.
• Manage visits, documentation, and meetings through effective time management and prioritization skills.
• Learn and utilize an electronic medical records system.
• Drive with reliable transportation, valid driver's license, and auto insurance to cover the service area.

Requirements

• Current Registered Nurse (RN) licensure in the state of Georgia.
• Works well within a team environment and collaborates daily with the interdisciplinary team.
• Willingness to drive and have reliable transportation.
• Must be able to prioritize and triage multiple concerns effectively.
• Must be willing to adhere to hospice rules and regulations.

Qualifications

• Previous hospice experience is required.
