Hospice Case Manager

Hospice Nurse provides compassionate hospice nursing care to patients and their families, coordinating individualized care to support comfort, dignity, and quality of life. Key Responsibilities Assess patient needs and provide direct nursing care in the hospice setting.Develop, implement, and update individualized care plans.Collaborate with physicians and interdisciplinary teams to ensure coordinated treatment.Document patient assessments, care provided, and plan updates accurately and timely.Support patients and families through education, guidance, and care coordination.Supervise home health aide services and ensure appropriate assistance with care needs. Qualifications & Skills Strong clinical assessment and nursing judgment skills.Ability to develop and maintain patient-centered care plans.Effective communication and collaboration with care teams and families.Proficient documentation practices and attention to detail.Experience in hospice/palliative care and ability to prioritize comfort-focused care.
