Schedule
Full time, Day Shift with every other weekend. 8:30am - 5pm.
Responsibilities
- Provides personal care to patients according to the assignment details and care plan.
- Documents patient care and observations timely and appropriately.
- Participates in end-of-life care for patients and families.
- Informs the registered nurse of any changes in the patient's condition.
- Follows the schedule of care, including making several visits per day with travel.
- Assists patients with activities of daily living, such as bathing, dressing, and grooming.
- Monitors and records vital signs, including temperature, pulse, and respiration.
- Supports patients with mobility and exercise as prescribed.
- Provides emotional support and companionship to patients.
- Maintains a clean and safe environment for patients.
- Communicates effectively with patients, families, and healthcare team members.
- Other duties as assigned.
Requirements
- High School Grad or Equivalent
- Experience in the health care field
- Hospice Aide training from an approved 75-hour training course and ongoing training
- Basic Life Support – CPR Certification (BLS)
- Driver's License and Auto Insurance
- Certified Nurse Assistant (CNA)
- Must maintain at least 12 contact hours of education annually

Pay
Pay Range: $15.15 - $22.73 per hour

This is a fieldwork position (Home and Facility Care) within Volusia County and some coverage in Flagler occasionally. Address: 770 W Granada Blvd, Ormond Beach, Florida 32174.
