Responsibilities

• Provides personal care and housekeeping assistance to clients in their homes, following the established plan of care under the direction of the client care coordinator.
• Delivers personal hygiene and grooming care to clients.
• Prepares meals to meet clients' nutritional needs.
• Reports changes in clients' condition to the client care coordinator or supervisor.
• Performs routine housekeeping and home management duties.
• Records each client's care on the care plan and submits it in a timely manner.
• Responds sensitively to the complex needs of terminal clients, including providing end-of-life care.
• Assists with routine therapy exercises.
• Participates in case conferences as requested.

Requirements

• High School diploma or equivalent (preferred).
• CNA Certificate approved by the Connecticut Department of Public Health and Addiction Services (required).
• Six months of experience as a Home Care Aide/CNA in a home care agency (preferred).
